The following will need to be completed for registration before student can participate in any sport:


1) FamilyID - go to FamilyID.com and register if you are a new student or if you already

have an account please update to current sport

2) Athletic & Activity Information Card

3) Current Physical and Medical Insurance Coverage Required

4) Pay $35 ASB Fee. Bring the completed Athletic & Activity Card, a copy of the FamilyID

verification email, and your $35 to the CVHS Business Office
